I, the undersigned parent or legal guardian of the participant(s) listed below, understand & agree to the terms outlined in this Liability Waiver. By signing this document, I acknowledge the legal implications & voluntarily assume responsibility for myself as well as the participant(s) named herein while attending any event associated with Oro Dance Competition (“ODC")

I release ODC, its organizers, staff, affiliates, & venues from all liability for any claims or damages arising from myself and my participant’s involvement in ODC events. I waive my rights and the participant’s right to take legal action against ODC for incidents, injuries, or losses associated with event participation, including travel to and from ODC events.

I accept & assume all risks of physical or psychological injury, illness, disfigurement, disability, medical bills, death, or economic loss related to event participation for myself, guests & participant. I accept responsibility for any injuries or outcomes resulting from the actions or negligence of participants, myself, others, the venue or competition.

I confirm that I’m responsible for the participant’s actions & obligations at all times. I agree to provide accurate information on this form & adhere to the rules and expectations outlined by ODC. I understand that by signing this document electronically, my signature carries the same legal effect as a handwritten signature.
